Program Description:
Formed in 1966 by neighbors in Portland’s Buckman neighborhood, community members came together to combat poverty and deteriorating conditions in Southeast Portland.
The agency was originally named Portland Action Communities Together, Inc. (PACT). Early Impact NW initiatives included employment programs, family counseling, food buying clubs, and a tool lending library. We helped develop Southeast Portland’s first Senior Center, Youth Service Center, and free health clinic.
In the 1990s, we piloted Multnomah County’s first Parent-Child Development Services program and the Schools Uniting Neighborhoods (SUN) program. Throughout our organizational history we have always aimed to help people thrive and remain safely housed.
The organization was renamed Portland Impact in the 1980s, and later Impact NW as we expanded our services outside the Portland Metropolitan area.
Transportation Services
Impact NW values the independence of seniors and adults with disabilities. In partnership with Multnomah County ADS, TriMet, and Ride Connection, Impact NW’s Transportation Assistance program makes it easy for our clients to get to medical appointments, do their shopping, and participate in social activities that keep them engaged in their community.
- 1Escorted Rides: Escorted door-to-door transportation services are offered to SE Portland seniors and adults with disabilities who need assistance getting to medical, dental and personal appointments.
- 2Bus tickets and/or taxi rides: Monthly TriMet bus/lift tickets are available for Impact NW senior and case management clients, based on qualification and availability.
- 3Group nutrition rides: Impact NW drivers provide group rides to major grocery stores fives times a week, as well as to select food banks. Food deliveries are provided to home-bound seniors.
- 4Recreational transportation: Impact NW drivers take groups of seniors and adults with disabilities on recreational trips planned by one of our activities coordinators.

Our mission is to offer opportunities for a life of dignity and hope to very low income seniors in Multnomah County by solving housing and transportation needs.
Our staff works with very low and extremely low-income seniors in Multnomah County to help them keep and find housing they can afford.
Thanks to the generosity of thousands of individuals and businesses, and through partnerships with area funders, we have provided housing placement, transportation and support to more than 40,000 older adults over the last 50 years. We believe housing is a human right and that every older adult deserves safe, decent housing they can afford.
Transportation Services provided: Our Transportation Services offer clients who are enrolled in our Housing Program with transportation to medical appointments, housing-related appointments, shopping or personal business. Our transportation services are designed to support Multnomah County seniors in maintaining their health and independence in permanent, affordable housing.

Ride Connection is a private, non-profit organization based in Portland, Oregon. We offer ways for people to access the goods and services they need to survive and thrive. By giving people the means to get around, we offer independence, health, and inclusion.
The Ride Connection network is made up of a collection of agencies who serve older adults and people with disabilities as well as low-income individuals and general public by offering a variety of transportation options in Clackamas, Multnomah, and Washington counties.
Together, we provide over 500,000 rides and support more than 2,000 individuals with training and access to public transportation each year.
Ride Connection and our partner agencies provide rides for any purpose including medical, meals, shopping, recreation, and volunteering or work. We’ll pick you up at your chosen destination, take you where you need to go, and pick you up when you’re done.
Our ride services are provided from your door to the door of your destination. In some cases, we may be able to offer more assistance and help with bags.

The Sandy Center offers a comprehensive range of advancement programs to enlighten, educate and enrich the lives of mature adults who want a broad spectrum of life experiences.
Programs include classes, lectures, forums, seminars and services to maintain the body, mind and vital aging.
Sandy City Transportation
For those adults age 60 and up, living in Sandy City or White City who do not have access to an automobile, the Sandy Parks and Recreation Department provides transportation Monday through Friday.

GroveLink is a free bus service in Forest Grove designed to help the community access employment opportunities, medical appointments, shopping, other local destinations and regional transit services: specifically TriMet bus line 57 and Ride Connection's WestLink Bus.

Program Description:

LTD offers FREE fare to our honored riders - older adults who are 65 years or better. Either use your Honored Rider card or photo I.D. as proof of age.
LTD offers half fare to qualifying people with disabilities. In order to establish eligibility for half fare, a person must provide a current, valid Medicare card, or proof of receiving SSI or SSD along with a valid photo ID card. Veterans who receive 50 percent or higher disability benefit are also eligible for half fare. Persons with disabilities who need an accessible accommodation in order to ride the bus may also be eligible for half fare.
Program Description:
El Programa Hispano is a social service nonprofit that serves the entire lifespan to address the social determinants of health. We have five main programming areas - education, from early childhood to
adulthood, housing, community wellness, UNICA, the first bilingual/bicultural domestic violence/sexual assault prevention and intervention program, and economic opportunities. We impact over 20,000 people a year in Multnomah, Clackamas, and Washington counties.
Senior Program:
Case management, mental agility, and social activities, physical activity, cultural enrichment, and a warm nutritious lunch provided to older adults Monday to Thursday, 10:00 to 12:30, This group is open to Multnomah county residents over age 60 (or adults with disabilities). Transportation can be arranged if qualified for medical transportation.

Program Description:
The program serves all Multnomah County residents living in or traveling to rural areas of the county, outside the TriMet service district. All rides must begin and end within Multnomah County.
Operated with funding from the state, we are contracting with Ride Connection for this service.

Program Description:
We’re launching a new transit shuttle service that will provide a vital connection to our rural communities and areas that are hard to get to by transit.
These shuttles will help relieve congestion on county roads when everyone returns to normal life after social distancing, and they will provide critical, last-mile connections between service provided by regional transit.
- 1The shuttles are a free “deviated fixed-route” service.
- 2Call 24 hours in advance to make arrangements and check availability to be picked up or dropped off within ¾ mile from the route.
- 3You may also signal/flag the bus to get on or off the bus in-between stops.
- 4Service Lines: The two shuttle services will fill gaps from TriMet stops to underserved portions of Oregon City and the Clackamas Industrial Area. Shuttle service will begin running by July 12, 2021.
- 5Oregon City: This shuttle line provides enhanced transit access throughout Oregon City. This shuttle operates on weekdays only. Service begins and ends at the Oregon City Shopping Center at 6 a.m. and the last scheduled stop is at 7:17 p.m.
- 6Clackamas Industrial Area: This shuttle line provides enhanced transit access to the Clackamas Industrial Area located primarily east of Interstate 205 along Highway 212. This service will link Clackamas Town Center Transit Center and locations throughout the Clackamas Industrial Area, ensuring connections to large employers in Clackamas and to Clackamas Community College.
The Clackamas Industrial Area shuttle operates all days of the week; weekend service is mornings only. Service begins and ends at Clackamas Town Center at 4:45 a.m. to 9:36 a.m. all days of the week, as well as 1:30 p.m. to 8:21 p.m. on weekdays. - 7Oregon City click here for full timetable
