The Canal Winchester “Welfare Corporation” was established in 1955 by a group of friends and neighbors who wanted to assist with emergency needs and provide holiday assistance to the residents of the Canal Winchester School District.
Senior Transportation
The Canal Winchester Senior Transportation Program provides a number of services for Canal Winchester residents who are at least 60 years old and live within the City's School District limits. These services are free due to grant funding provided by local funders*. From medical appointments to Senior Citizens Club activities, we are dedicated to improving the quality of life, health, and well-being of our residents.
The Program includes both Daily Transportation and Group Transportation. Daily transportation is provided for both medical and personal appointments, with priority given to medical appointments. Examples of personal appointments include beauty appointments or trips to the local library. Group transportation includes grocery trips to four local groceries and a variety of pre-arranged Senior Citizens Club activities.

Program Description:
Groveport Senior Transportation truly wishes to assist seniors and individuals with disabilities maintain the independent lifestyle they desire. For these residents, we provide free door-to-door transportation to medical and personal appointments.
- 1The City of Groveport is pleased to provide this service free of charge to qualifying residents.
- 2We will schedule a home visit to complete your sign up and orientation. We are typically able to schedule your visit within 5-7 days.

Program Description:
The Community Services Department is pleased to provide these services for our senior residents. We recognize that many Bay Village residents wish to remain in their homes as long as they possibly can.
Our goal is to provide programs and services to help make this possible.
Funding: There is no federal government funding for the operation of this program. All operating expenses are paid by the City of Bay Village. Financial support from the riders is necessary. Please pay your fair share.

Established in 1988, Bikur Cholim is dedicated to serving all members of the Jewish community by providing assistance to those experiencing health-related problems.
We strive to help young and old, from within Cleveland and beyond, by offering comfort, shelter and support through a variety of services.
We accomplish this through the coordinated effort of dedicated volunteers who selflessly perform the mitzvah of "bikur cholim" - literally "visiting the sick" - with all of the nuances of care and concern which this community embraces. Bikur Cholim sees its duty to stand with individuals and their families during times of medical crisis in accordance with Jewish tradition, as are the practices of the organization.

Program Description:
Free door-to-door wheelchair lift equipped van service is available for Beachwood residents 60 years and older and those with disabilities living in an independent living setting.
This service is not meant to supply all transportation needs, but is a supplemental service. Rides are available for medical appointments, shopping, banking, and social / recreational activities. There is no guarantee of the van's availability for requested dates and times.

The Division of Aging was established in 1973 to provide a continuum of responsive services to Lakewood’s older residents.
Programs and services evolved through the years, contributing to the Lakewood Division of Aging becoming one of the largest municipal aging programs in Ohio. The Division underwent some reorganization in 2008 with changes in programs and services. However, the primary goal of the Division’s core programs remains the same: enabling older residents to stay safely in their homes and community while sustaining a quality of life that is both meaningful and productive.

The Office on Aging serves thousands of residents age 60 and over, every year through information referral and supportive services. Programs such as Meals on Wheels of Cleveland Heights, Inc. prove invaluable to the senior community.
Senior residents are also assisted by Office on Aging social workers, and the new Senior Activity Center at the Cleveland Heights Community Center, located at One Monticello Boulevard, has programming and events for a wide range of interests.

Seniors 60 years or older and physically challenged residents who live in the Village of Oakwood.
ALL riders must be able to board the vehicle unassisted, or provide an aide to accompany them. Wheelchair accessible vehicles are for common wheelchairs. Drivers will assist riders on and off the wheelchair lift and tie down the wheelchairs.

Program Description:
The Elderly Services Program (ESP) helps older adults from Cincinnati to Oxford, Mason to Wilmington, and everywhere in between remain safe and independent in their homes by providing senior home care services such as personal care, housekeeping, meals, transportation and more.
ESP expands care seniors may already receive from family and friends and prevents unnecessary nursing home placement. COA administers the Elderly Services Program in Butler, Clinton, Hamilton and Warren counties. These programs are funded by county tax levies.

Council on Aging (COA) is a nonprofit organization dedicated to enhancing quality of life for older adults, people with disabilities, their families and caregivers.
We promote choice, independence, dignity and well-being through a range of services that help people remain in their homes for as long as possible.
Founded in 1971, COA is part of the national aging services network and is a state-designated Area Agency on Aging, serving individuals across a multi-county region. Our core service area includes Butler, Clermont, Clinton, Hamilton and Warren counties, with limited programming in Greene and Montgomery counties. One call to COA links people to the wide variety of agencies, information and programs that serve older adults, people with disabilities and their caregivers.
As an Area Aging on Agency, we are responsible for planning, coordinating and administrating local, state and federally-funded programs and services for older adults, people with disabilities and caregivers in our planning and service area: Butler, Clermont, Clinton, Hamilton and Warren counties.
PASSPORT is a Medicaid program that helps older adults with disabilities receive care and support in their homes, instead of a nursing home.

Hamilton County Job and Family Services administers state, federal and local programs designed to help those in need and help families work toward self sufficiency.
The agency has served Hamilton County since 1947. Today, the approximately 850 employees that work for the agency serve hundreds of thousands of Hamilton County residents.
The department is one of the few quadruple-combined public human service agencies in Ohio – providing public assistance, children’s services, child support and workforce development programs to the community.
Among the department’s many duties: local child protection, adult protection, child care, child support enforcement, workforce development, cash assistance, food assistance and medical assistance.
HCJFS is the largest department within Hamilton County government and reports to the Board of Hamilton County Commissioners.
Non-Emergency Transportation (NET) is a statewide program administered by Hamilton County Job and Family Services for Medicaid eligible customers. NET provides transportation to and from Medicaid providers. Transportation authorization may be in the form of bus passes, van services, Access tickets or mileage reimbursement. NET Requests must be submitted five days before transportation is needed.

The J’s Transportation Services allows senior adults to move freely around their community, giving them more independence and greater access to activities that contribute to a better quality of life.

- 1We provide daily transportation for seniors to go shopping and visit the JCC. Every weekday, seniors are picked up at their homes and transported to the J for fun activities, wellness programs, trips, and lunches, and then returned home.
- 2Through our Non-Emergency Medical Transportation service, seniors are picked up at their homes and driven to their medical appointments, including therapy, dentist, dialysis, testing, X-ray, and more. Participants can also be transported to the pharmacy to pick up prescriptions.

To benefit the greater community, The North College Hill Senior Center provides opportunity for friendship, recreation, wellness and education, which enable Senior Citizens to maintain an active quality of life.

The Toledo Area Regional Transit Authority (TARTA) provides a vital link in the Toledo metro area to jobs, education, health care and other destinations.
It enables everyone in our community to have access to transportation regardless of their age, race, physical ability or economic background.
TARTA serves seven communities in Northwest Ohio: Maumee, Ottawa Hills, Rossford, Sylvania, Sylvania Township, Toledo, and Waterville.
TARTA is underway with exploring and introducing new initiatives and service options that will enhance convenience and customer experience while moving Toledo into the future of public transit.

Program Description:
PASSPORT is for seniors age 60 and older who need the level of care provided in a nursing home, but want to stay in their home. It is a statewide program funded by Medicaid.
Those who qualify for PASSPORT work with a nurse or social worker to develop a plan of care that customizes services to help them remain at home. Once enrolled, a case manager works with them to be sure needs are met with quality services, and that the plan of care is adjusted as needs change.
- 1There is no charge for an in-home assessment to discuss your needs and options.
- 2There is no charge for PASSPORT services if you are under a set income level. If you are over the income level, you may choose to pay a monthly amount determined at the time of enrollment or decline enrollment.
- 3PASSPORT is funded by a Medicaid Waiver that differs from traditional Medicaid, and therefore you may qualify for Medicaid even if you have been denied in the past. If you provide income and asset information to our assessor, a preliminary determination can be made during the in-home assessment. The assessor can also assist in completion of the Medicaid application, but you must follow up with the application process through the Department of Job and Family Services.

The Parks and Recreation Department offers a free transportation service to Twinsburg residents 65 and older.

UDS Transportation Services operates one of the largest fleets of accessible vehicles in Summit County, providing safe and
reliable transportation for our passengers in need of Non-Emergency Transportation (NET) for medical appointments. Our lift-equipped vans are also available to community organizations for special events on a contract basis.

Hilliard residents over the age of 55 can ride the Hilliard Express safely and free of charge to popular destinations.
The rideshare service will pick seniors up from their private homes or living communities and take them to dining, shopping, entertainment, parks, medical facilities and places of worship. Set locations such as specific restaurants and stores are available, but riders may request unique destinations within the city.
All Hilliard Express vehicles are equipped with professional drivers and access for wheelchairs. While on the shuttle, other riders may be picked up. COVID-19 guidelines will be strictly enforced.

Life Care Alliance offers seniors 60 and better an opportunity to eat and socialize with other seniors. Clients may come Monday through Thursday from 10:00am-1:00pm to enjoy socialization and hot/cold lunch year round for a donation or for free.

Senior Transportation Connections
A simple transportation service to help seniors get to the Senior Center, local physicians' offices, banking and shopping is available to Orange School District seniors ages 60 and over and to disabled adults, and is made possible through financial support of the communities of Hunting Valley, Moreland Hills, Orange Village, Pepper Pike and Woodmere, private donations, and a grant from the Ohio Department of Aging through the Western Reserve Area Agency on Aging.
- 1Transportation is provided by Senior Transportation Connection. Service normally runs Monday to Friday, 8:30 am. to 4:00 pm.
- 2Each client may take 12 one-way trips or 6 round trips each month. Please notify Senior Transportation Connection when booking your trip if you will be taking a wheel chair or companion. Companions ride free of charge.
- 3A donation of $3 for a one-way trip or $6 per round trip is recommended to help off-set the cost of this service.
- 4Call at least 3 days and up to 2 weeks prior to your appointment to schedule your trip.

West Side Community House (WSCH) was founded in 1890 as the Methodist Deaconess Home of Cleveland. WSCH has operated as a private, nonsectarian nonprofit 501(c)3 social service agency since 1964, owned and operated by a board of managers.
We work to meet the needs of community residents by providing an array of model programs and services for children, families and older adults. Services are open to all social, racial, ethnic and income groups but are targeted to those who are most vulnerable and at-risk.
Our programs strengthen the family, provide opportunities to learn and grow, support independent living, encourage economic self-sufficiency, and renew the life spirit.
Healthy Aging Center: At West Side Community House's Healthy Aging Center, our mission is to promote healthy aging and link people with resources so they may continue to lead happy, rewarding and fulfilling lives in the community.

Program Description:
In Northeast Ohio, the Old Brooklyn neighborhood is home to one of the largest populations of residents 60 years and older.
While many of these seniors still care for themselves, many rely on charitable organizations for ongoing support and opportunities to stay active within the community.
Since 1971, Senior Citizen Resources has filled this need. Our programming helps to keep individuals healthy and living independently by meeting their basic needs. Our main programs — transportation, nutrition, activities, social service, and volunteering — focus on health and wellness, and provide opportunities for creativity, lifelong learning, and socialization.
To date, we have positively impacted the lives of thousands of seniors, while continuing to expand our services and broaden the reach of our programs.

Each year, over 40,000 older northwest Ohioans are served through the Area Office on Aging and its network of 180 providers.
Lucas County Senior Dining Site Program
Lyman W. Liggins Senior Nutrition Program provides a home-style lunch meal Monday-Friday at various dining sites throughout Lucas County. The senior dining program provides an opportunity to enjoy a good meal with friends in a safe, friendly environment. Nutrition education, socialization and other activities are available at the dining sites.
For those with limited transportation, a TARTA bus pass may be available to travel to and from the dining site. In addition, a daily (Monday-Friday) hot meal or once a week delivery of frozen meals may be delivered to home-bound individuals 60 years of age or older. Each home-delivered and congregate meal provides one-third the daily recommended key nutrients for individuals 60 years of age and over and includes low-sodium and diabetic-friendly options.
