Program Description:
Currently, Canaan Baptist Church of Christ is the sponsor and contractor for the Canaan Senior Service Center, which provides services and nutritional aid for the senior residents of the surrounding community.
Located in Harlem, the center has become an important organization in the community through its work with the senior citizens it serves and assists. Through the visionary leadership of Dr. Wyatt Tee Walker, the Canaan Senior Service Center began in 1972 with a one-day-per- week program totally funded and staffed by volunteers of Canaan Baptist Church.
Since September 1975, the center has continued to be sponsored by the church, with a monthly contribution, but is funded by the New York City Department for the Aging (DFTA) for personnel services, meals, rent, utilities, raw foods, and other expenses such as consultants and Title V senior program aides. Personnel funds are provided from four sources, DFTA, Title V, Eastern Seals and Angel Guardians for the Elderly.
Canaan Senior Service Center staff provides daily comprehensive services Monday through Friday from 9:00 am. to 5:00 pm. These services include case assistance, entitlements counseling, information, referrals, escort services, shopping assistance, transportation, telephone reassurance calls to the isolated and home bound, friendly visits to the sick and shut-in, and education and recreation for the daily participants.

Program Description:
Columbia University provides free shuttle bus service for qualifying individuals via the ADA-accessible Intercampus Shuttle. The shuttle connects 96th Street, the Morningside campus, the Medical Center (CUMC), and Harlem Hospital (HH) with stops at nearby subway and transit stops.
Shuttle service runs via the Columbia Intercampus Shuttle. Look for the Columbia Community Shuttle decal next to shuttle doors. This identifies the Intercampus Shuttle for your use.

We serve our clients with a diverse range of programs and services.
We deliver those programs and services through intergenerational connections with volunteers of all ages.
In 1976, DOROT’s founders set out to meet the needs of their senior neighbors who were hungry for both food and social connection. As recent graduates of Columbia University’s social work program, they were pioneers in their approach to serving older adults.

Program Description:
Celebrating over 15 years serving the Lower Manhattan and Battery Park City communities, the Downtown Connection is New York City’s only free circulator bus service.
The Downtown Connection features five state-of-the-art buses offering free service across Lower Manhattan and Battery Park City, each with seating for 22 passengers. All buses are easily identifiable by their red color and Downtown Connection branding.
The Downtown Connection has also made accommodations for Covid safety protocols, such as updated air-filtration systems, plastic partitions installed between seats and forward-facing seating for additional passenger protection.
With easy access to subway lines, bus routes and ferries, the Downtown Connection is the most convenient way to explore Lower Manhattan’s businesses, shops, restaurants and attractions. Bus rides are free, so hop on and off as often as you’d like.

Program Description:
Encore Community Services was founded in 1977, by Sr. Lillian McNamara, Sr. Elizabeth Hasselt and Fr. George Moore to provide food, socialization and support for the elderly of the Hell’s Kitchen and Broadway communities.
What started as a single community center in the basement of St. Malachy’s Church has since grown into a multipurpose, nonsectarian agency providing a comprehensive array of programs and services to elderly New Yorkers. Each year Encore provides over 400,000 meals; both on-site, at our senior center and delivered directly to our homebound members.
In addition to providing meals and social services to the elderly, Encore also operates the Encore 49 and Encore West Residences. The Encore 49 Residence provides an SRO-style supportive housing facility for formerly homeless seniors with special needs. In a safe, clean and caring environment, residents are provided with their own units, and a range of mental health services. The Encore West Residence provides 84 independent housing units for the very low-income elderly.
Encore strives to provide a safe and welcoming environment to any senior that may need us, from meals, to social services, to housing; Encore’s philosophy remains the same: “By nurturing, respecting and enabling, Encore hopes to improve the quality of life of older New Yorkers in need, in an approach that emanates from the core of Encore’s commitment: we believe that what we do comes from the heart and the heart is the center of all.”
Encore also reaches out to elderly who are homebound, who no longer are able to make the trip to a senior center, or who have little or no access to support services. By providing a comforting voice over the phone, or a friendly face who pays a visit, outreach efforts may enable a senior to remain at home and also assists with reinforcing independence.
The potential effect of this may be to lessen the chance of premature institutionalization, allowing the senior to remain “at home” with needed social supports.

Program Description:
Senior Companions are healthy older adults who volunteer to assist homebound or isolated fellow seniors live independently. Volunteers are matched with a client for regularly, weekly, scheduled companionship services.
These services include: friendly visiting, shopping assistance, escort to a doctor’s appointment or other community activity, help with obtaining services such as access-a-ride and meals on wheels, and most importantly, friendship and companionship.
Sponsored by Henry Street Settlement and funded by Americorps Seniors, the Senior Companion Program is available in all of New York City’s five boroughs.
- 1In response to the COVID-19 pandemic, the Senior Companion Program Volunteers are not currently conducting in-person companionship services for clients. All services are provided to clients over the phone or via virtual programming. We are still accepting new potential volunteers and potential clients, but services are limited until further notice.

New York Foundation for Senior Citizens (NYFSC) is dedicated to helping New York’s seniors enjoy healthier, safer, more productive and dignified lives in their homes and communities and to help them avoid the need for premature institutionalization.
Established in 1968, NYFSC is the only non-profit, non-sectarian organization serving New York’s seniors in all five boroughs.
NYFSC continues to grow. From modest beginnings of one social worker and one bookkeeper, the organization now has more than 1,500 employees to address the needs of the City’s seniors. NYFSC manages 35 social service programs, in addition to more than 900 units of housing. This important work is made possible with the leadership of our Board and the amazing efforts of NYFSC’s staff.
NYFSC’s programs are funded by Federal, State, and City governments and by private contributions. Supporters are confident that NYFSC will continue to achieve the best outcomes for the seniors through its commitment to achieving results, keeping overhead low and carefully managing budgets.

DAV operates a fleet of vehicles around the country to provide free transportation to VA medical facilities for injured and ill veterans.
DAV stepped in to help veterans get the care they need when the federal government terminated its program that helped many of them pay for transportation to and from medical facilities.
The vans are driven by volunteers, and the rides coordinated by more than 155 Hospital Service Coordinators around the country.
Since the program’s inception in 1987, DAV departments and chapters have donated 3,558 vehicles and Ford Motor Co. has donated 239 vehicles at a cost of nearly $89 million.

We will strive to provide, assist and coordinate community resources to implement services and programs for youth, adult, and senior groups, related to the well-being of the City's residents in a professional, efficient, and courteous manner.
"GOING PLACES" VAN SERVICE: This service is for City of Tonawanda residents only, 60 years of age or older, who are capable of getting in and out of the van without assistance. This service is made possible by the City of Tonawanda, Erie County Department of Senior Services, and the NYS Office for the Aging.
- 1The van is available Tuesday through Friday beginning at 7:30am. Last pick up is 2:30pm.
- 2We take request for transportation every Monday morning for appointments that week.

The Clarence Senior Center enriches the lives of seniors in our community through a wide variety of activities, programs, classes and experiences that will educate, inform, entertain and involve them.
Seniors age fifty-five and over are welcome regardless of race, creed, ethnicity, or nationality. With nearly 1000 members, the Senior Center is a busy place where members can come for an hour or stay for the day and enjoy all that we have to offer!
The Town of Clarence provides a vehicle for Center use. Its main purpose is to provide transportation for members to and from the Center.

The Erie County Department of Senior Services was created in 1971 and subsequently designated an Area Agency on Aging under Title III of the Older Americans Act.
The primary responsibilities which the Department has under the Older Americans Act and New York State Community Services for the Elderly Programs focus on planning, advocacy, and coordination activities.
These responsibilities are:
To develop and implement a four-year plan of services which is responsive to the priority needs of older persons in Erie County. Services under this plan are delivered either directly by the Department or through community organizations under contract with the Department
To advocate on behalf of the needs of older persons by monitoring, evaluating, and commenting on policies, programs, and community actions which affect the elderly in Erie County
To represent the needs, interests, and concerns of older persons to public officials and others
To coordinate our planning and service activities with other agencies and organizations to promote the most efficient use of existing resources and to promote new and expanded benefits to the elderly.
Transportation: As a service to older adults (aged 60+) in Erie County, in collaboration with community partners, the Department of Senior Services offers the Going Places Transportation Program.
The curb-to-curb van service is available to seniors who are unable to use conventional means of transportation.

- 2Because of the demands for senior transportation, travel may be limited to the nearest available facilities (with the exception of trips for health purposes).

The City of Buffalo Department of Community Services and Recreational Programming is comprised of seven divisions that invests in a network of community-based organizations and programs to enrich and complement the lives of Buffalo residents, particularly city youth and city seniors.
By providing and funding a wide range of high-quality recreation and leisure opportunities, we support youth and seniors by meeting their social, economic and physical needs while promoting diversity, cultural, neighborhood, family, education, recreation and wellness opportunities.

- 2You must be ready to return, (return trip) no later than 2:30 p.m.

The Grand Island Golden Age Center provides services to all Grand Island residents, 50 years of age or older, regardless of religious, racial, political or ethnic background.
This Town-sponsored facility provides transportation services, special events, educational, recreational & travel opportunities, as well as a congregate site featuring home-cooked meals weekdays.
Our mission is to provide safe, nurturing, and enjoyable activities to meet the needs of our senior population.
Membership is free of charge.

The Center is open 8 am - 4 pm Monday through Friday.

Program Description:
Hearts and Hands is the premier volunteer organization serving our frail or vulnerable neighbors in rural and suburban Western New York communities.
For too many of our older neighbors in Erie and Niagara Counties, losing the ability to drive means losing access to critical health care, necessary grocery trips and much-needed social interactions. Additionally, people who live below the poverty level, have Medicaid, or live in rural areas have a more difficult time accessing medical care and healthy food.
Through its transportation services Hearts and Hands increases access to medical care, food, and opportunities for physical and social activity.
We accomplish this by providing transportation that meets the unique needs of older adults living in the suburban and rural areas of Erie and Niagara Counties. Volunteers use their personal vehicles to provide door through door escorted transportation for our care receivers.
Our care receivers do not need computers, smart phones or internet access to request services.

Program Description:
Today, MAS currently manages Medicaid transportation for approximately 5,100,000 Medicaid enrollees in New York State, which is approximately 7 percent of the national Medicaid population (source: Kaiser Family Foundation).
MAS takes a team approach including assigning regional Medicaid administrators, assistant regional Medicaid administrators, field liaisons, bus pass and mileage reimbursement administrators, as well as commercial travel and quality assurance staff directly to each region and to specific counties within each region it manages. MAS works closely with medical practitioners and transportation providers to ensure that Medicaid enrollees are provided courteous and professional transportation.
In 2004, MAS managed Medicaid transportation for the Onondaga County Department of Social Services. Over the next six years, MAS added Monroe, Sullivan, Niagara, Oneida, Dutchess, Orleans, Ulster, and Cayuga to its portfolio.
In 2011, New York State redesigned its Medicaid program, dividing the state into six regional initiatives.
MAS won the first contract awarded under this new structure, and signed on to provide non-emergency Medicaid transportation (NEMT) in the Hudson Valley region.
In 2013, MAS was awarded the Medicaid transportation management contract for the 24 county Finger Lakes Northern NY region.
The WNY region was awarded to MAS in 2014.
In 2016 MAS was awarded second five-year contract to manage Medicaid transportation for the Hudson Valley region.
In 2017, MAS was awarded a five-year contract for Medicaid transportation management services in New York City.
The four New York State regions for which MAS currently manages Medicaid transportation for NYSDOH includes approximately 5,100,000 Medicaid enrollees, thousands of medical practitioners and over a thousand transportation providers.

The Northwest Buffalo Community Center Inc. (NWBCC) was founded in 1975 and is one of the largest human service providers in the greater Buffalo area.
Programs offered range from complete medical and dental

assistance, daycare and much more. Our staff is trained to offer assistance in all areas of need, including the ability to make referrals to appropriate agencies, often to programs already on site a 155 Lawn Avenue.
The service programs of the Northwest Buffalo Community Center are
governed by a volunteer board of directors made up of local citizens,
business leaders and government officials who together make sure that you come first and the services you receive are handled in a competent and professional manner.

Program Description:
Rural Transit Service is a non-profit transportation organization. Rural Transit Service provides safe, affordable and reliable transportation to seniors, low income and disabled clients to allow them to maintain a happier, healthier more independent lifestyle and heighten involvement in the community.
Eligible residents are transported to dialysis clinics, medical offices, physical therapy, pharmacies, colleges, work places, nutrition sites, adult day cares, grocery stores, banks, salons/barbers, etc., with medical trips taking priority.
Rural Transit’s success is predicated on the commitment of their volunteers, including drivers, dispatchers and driver aides; providing the transportation and scheduling. In twenty five years, due to the increasing need, Rural Transit has expanded its service area from two towns, with approximately one dozen volunteers to twenty five municipalities throughout Erie County with the help of over one hundred thirty volunteers and the need continues.

Program Description:
The Valley Community Association Senior Citizen Program is designed to encourage and maintain healthy lifestyles in elderly residents of the Buffalo River Community by providing affordable healthy meals, facilitated physical activity,
peer interaction and socialization, and health care navigation and access to support services. Program activities include congregate dining, low-impact aerobics exercise, cards and BINGO, healthy aging education, recreational activities, day trips, and more.
Transportation is also provided for those in need. Through collaborations and partnerships, the program also gives the local senior population access to regular on-site free screenings and consultations. The VCA Senior Program is dedicated to promoting preventative measures and provides a network of support to improve the quality of life in the local senior population.

FISH receives calls from individuals unable to drive themselves to essential appointments at hospitals, doctors’ offices, and medical specialists.
Clients should be able to walk from their home to the vehicle and in/out of the appointment facility – independently or on the arm of a driver. FISH serves residents who live within the Rush-Henrietta Central School District.

Program Description:
The Fairport/Perinton Senior Living Council, Inc. (SLC) partners with the Fairport Baptist Homes (FBH) to provide the transportation options offered through Senior Options For Independence (SOFI).
The Elderbus provides door-to-door, wheelchair accessible transportation Monday through Friday within the Fairport/Perinton area for residents who are at least 60 years of age or disabled.
Through a partnership with the Fairport Baptist Homes, SLC makes it possible for Fairport/Perinton residents to get to local doctors’ appointments, go to the library, have their hair done, grocery shop, and do their banking while socializing along the way.

Program Description:
RTS Access is RTS’ paratransit program that provides coordinated ride-sharing transportation for people with disabilities who are unable to ride RTS transit buses due to their disability.
RTS Access is a complementary service, meaning it matches the service area and operation hours of RTS' system, and is provided under the criteria set forth by the Americans with Disabilities Act (ADA) of 1990. As an origin-to-destination service, RTS Access routinely provides curb-to-curb service with exterior door-to-exterior door service provided upon advanced notice.
- 1The RTS Access service area is ¾ of a mile beyond the RTS fixed-route bus service.
- 2RTS Access trips must begin and end within the service area as defined by the ADA.
- 3Reservations can be made up to a maximum of seven days in advance.
- 4When reserving a return trip from a medical appointment, remember that medical appointments are rarely completed on time. Please allow additional time for your requested return pickup time. Paratransit requires a minimum stay of one hour after a drop-off at any destination.

The Webster Association of Senior Program Supporters was started in August 2010 by John (Bud) Johnston with the intention of giving a neighbor a ride to a medical appointment.
It has blossomed into a booming organization that has supported hundreds of seniors who live in the Webster School District.
Wasps (Webster Association of Senior Program Supporters) is a group of volunteers who provide transportation for retires and disabled residents who live within the Webster School District.
Our Mission is to help the retired and disabled residents in our community get to and from medical and dental appointments, other health related trips such as pharmacies, as well as banks, barbers, hairdressers and tax prepartion services.

RideConnect has mobilized a team of volunteers willing to shop for, and deliver groceries to older adults (60+) and those with compromised immune systems, living in Westchester and Putnam Counties. Our volunteers are eager to help those at highest risk for Covid-19.
Our goal is the safety of everyone, so our volunteers practice social distancing, and all CDC recommended hand washing practices. Volunteers will not transport a client, and they will not enter a home.

A bus service is provided for eligible Cortlandt Seniors for shopping trips to the Cortlandt Town Center and to the Jefferson Valley Mall. Seniors are picked up at their homes and returned back to their homes following the shopping trips.
The shopping bus goes to the Cortlandt Town Center every Thursday, and to the Jefferson Valley Mall the 3rd Monday of the month.
